The dreaded client intake form. Everyone hates filling them out. It’s like asking your life story along with your deepest, darkest secrets. Why do we have to fill them out? What’s the point? Do you really need all of this? Who cares? It’s JUST a massage.
WRONG! Super wrong. If none of that information was important I wouldn’t be asking you to fill out the form. Yes, they’re long, detailed, and a pain to fill out. None the less, it’s all important and it is even more important that you fill it out completely and honestly. If you feel that there is something that you don’t want to fill out because you’re afraid that I won’t do the massage then you may need to reschedule. The intake is so important. It is not only for my safety, but mainly for YOUR safety. If for some reason I need to call an ambulance, I can’t stand there with a blank face and tell the paramedics, “I have no idea who this person is.”
There are many contraindications (condition that would be reason for you to not be able to receive a service) with massage. If the form is not filled out completely or properly, there could be things that could make the massage session cause more harm than good. The last thing anyone wants is to have a bad massage. Medications and health problems may not mean that you can’t get a massage at all, but it could mean that the massage would need to be altered for your needs.
So, what WOULD happen if someone decides not to fill out the intake form? If the intake form is not completed and signed, I will not perform the massage. Before your first massage with me, please come prepared to fill out the paperwork. Also, be prepared to fill one out periodically or should anything change. If anything does change, please let me know before your next massage.
